ABSTRACT

RESUMO. Este estudo, fundamentado na perspectiva da psicologia cultural-histórica sobre a pessoa com deficiência, teve por objetivo apreender a dimensão subjetiva da realidade (ou as mediações) das crianças com a Síndrome Congênita do Zika Vírus (SCZV) no contexto escolar de desenvolvimento e aprendizagem a partir dos sentidos produzidos por cuidadoras escolares. Para tal, foram realizadas entrevistas semiestruturadas com três cuidadoras escolares que trabalham em três creches pertencentes ao sistema de educação de ensino de Campina Grande/PB. Para análise dos dados, foi realizado o procedimento dos Núcleos de Significação, que visa à apreensão das contradições que constituem as produções de significação discursiva dos sujeitos participantes. Os resultados indicaram que as cuidadoras escolares priorizam a mediação pedagógica na relação estabelecida com as crianças com SCZV, embora não desconsiderem a instância do cuidado em termos das necessidades especiais relacionadas à integridade psicomotora que essas crianças apresentam. Ademais, foi evidenciado que as participantes salientam as potencialidades das crianças em detrimento da falta ou lesão gerada pela deficiência.


RESUMEN. Este estudio, basado en la perspectiva de la psicología cultural-histórica sobre las personas con discapacidad, tenía como objetivo apreciar la dimensión subjetiva de la realidad de los niños con Síndrome Congénito del Virus del Zika (SCVZ). en el contexto escolar del desarrollo y el aprendizaje de los significados producidos por los cuidadores escolares. Para ello, se realizaron entrevistas semiestructuradas con tres cuidadores escolares de guarderías diferentes que pertenecen al sistema educativo de Campina Grande/PB. Para el análisis de datos, se realizó el procedimiento de los núcleos de significación, cuyo objetivo es aprehender las contradicciones que constituyen las producciones de significado discursiva de los participantes. Los resultados indicaron que los cuidadores de la escuela dan prioridad a la mediación pedagógica en la relación establecida con los niños con SCVZ, aunque no descuidan la instancia de cuidado en cuanto a las necesidades especiales relacionadas con la integridad psicomotora que tienen estos niños. Además, se destacó que los participantes ponen de relieve el potencial de los niños en detrimento de la falta o lesión generada por la discapacidad.


ABSTRACT This study is based by the perspective of the cultural-historical psychology on people with disabilities, aimed to apprehend the subjective dimension of the reality (or mediations) of children with Congenital Zika Virus Syndrome (CZVS) on the school context development and learning from the senses produced by school children caregivers. For this reason, semi-structured interviews were conducted with three caregivers working in three daycare centers belonging to the teaching system of education in Campina Grande/PB. For data analysis, was performed the meaning core, which aims to apprehend the contradictions that constitute the productions of discursive meaning in the participating subjects. The results indicated that school caregivers prioritize the mediation in the relationship established with children CZVS, though not disregard the instance of care in terms of the special needs related to psychomotor integrity that these children have. Furthermore, it was evidenced that the participants emphasize the children's potentialities to the detriment of the lack or injury generated by the disability.

INTRODUÇÃO: No segundo semestre do ano de 2015 um surto de microcefalia foi registrado na região nordeste do Brasil. O presente artigo tem como objetivo traçar o perfil clínico dos portadores da síndrome congênita do zika vírus da cidade de Campina Grande. MÉTODOS: Tratou-se de um estudo documental com abordagem quantitativa. A coleta de dados deu-se no Centro Especializado em Reabilitação (CER), no município de Campina Grande no estado da Paraíba com análise de 66 prontuários. RESULTADOS: Maioria do sexo feminino (57,6%), com idade média de 3,36 anos. Houve predominância de infecção pelo vírus da Zika no 1º trimestre de gestação (69,7%), em mães com baixo grau de escolaridade em que apenas 28,8% completaram o ensino médio. A maioria das crianças, mesmo com a infecção, nasceram a termo (68,2%), porém, apresentaram baixo peso ao nascer, 2,697kg em média, perímetro cefálico médio de 29,97 cm, hipertonia, diminuição da amplitude de movimento, grande dependência nas atividades de vida diária e um importante atraso no desenvolvimento neuropsicomotor. CONSIDERAÇÕES FINAIS: Com isto é possível destacar que os distúrbios apresentados, contribuem de maneira negativa para o desenvolvimento motor das crianças gerando um impacto em sua qualidade de vida


INTRODUCTION: In the second half of 2015, an outbreak of microcephaly was recorded in the northeast of Brazil. This article aims to outline the clinical profile of patients with congenital Zika virus syndrome in the city of Campina Grande. METHODS: This was a documentary study with a quantitative approach. Data collection took place at the Specialized Center for Rehabilitation (CER), in the city of Campina Grande in the state of Paraíba, with the analysis of 66 medical records. RESULTS: Most of them were female (57.6%), with an average age of 3.36 years. There was a predominance of Zika virus infection in the first trimester of pregnancy (69.7%), in mothers with a low level of education, in which only 28.8% completed high school. Most of the children, even with the infection, were born at term (68.2%), however, they had low birth weight, 2.697 kg on average, average head circumference of 29.97 cm, hypertonia, decreased range of motion, great dependence on activities of daily living and an important delay in neuropsychomotor development. FINAL CONSIDERATIONS: With this it is possible to highlight that the disorders presented, contribute negatively to the motor development of children generating an impact on their quality of life

ABSTRACT

RESUMEN El virus del Zika es transmitido por mosquitos Aedes aegypti y Aedes albopictus, se caracteriza por exantema, cefalea y fiebre y otras complicaciones. El primer caso del virus del Zika fue detectado en Cuba en febrero de 2016 y aunque está controlada la epidemia se mantiene el riesgo por existir presencia del vector. Con todos estos elementos los autores deciden realizar este estudio con el objetivo de determinar la influencia de las determinantes sociales en el comportamiento y la propagación de la enfermedad en América Latina. La declaración por la Organización Mundial de la Salud como una emergencia sanitaria hace necesario el desarrollo de un programa integral, para la erradicación del vector, la preparación de los profesionales actuantes y la información a la población que controle la propagación de la enfermedad (AU).


SUMMARY The Zika virus is transmitted by Aedes aegypti and Aedes albopictus mosquitoes; it is characterized by exanthema, headache, fever and other complications. The first Zika virus case in Cuba was detected in February 2016, and although the epidemic is controlled the risk still exists because of the presence of the vector. With all these elements the authors decided to carry out this study with the objective of determining the influence of social determiners in the behavior and propagation of the disease in Latin America. Its declaration by the WHO as a health emergency makes it necessary the development of a comprehensive program to eradicate the vector, to prepare the officiating professionals and to inform the population for controlling the disease spreading (AU).

ABSTRACT

ABSTRACT Objectives: to identify elements that contribute to strengthen the family system of children with Zika virus congenital syndrome according to Betty Neuman's theory. Methods: qualitative research, carried out in the outpatient service of a public hospital in the city of Recife, Brazil, with 13 mothers, by semi-structured interviews. The IRAMUTEQ software was used for data analysis and the interpretation was carried out according to Betty Neuman's Systems Model Theory. Results: the dendrogram originated five categories, which we named: Family Routine, Health Service Assistance, Changes in Lifestyle, Support Network, and Social Repercussions of Care for the the Family Context. Final considerations: nursing actions based on Betty Neuman's theory provide subsidies for the recognition of elements that strengthen the defense lines of the family system. These resources can be explored, aiming to maintaining the well-being and balance in the context of the family system.


RESUMEN Objetivos: identificar los elementos que contribuyen al fortalecimiento del sistema familiar del niño con el síndrome congénita del virus del Zika bajo la teoría de Betty Neuman. Métodos: investigación cualitativa, realizada con 13 madres en el ambulatorio de un hospital público de Recife, y en la que se utilizó entrevista semiestructurada. El software IRAMUTEQ se utilizó en el análisis de datos, y la Teoría del Modelo de Sistemas de Betty Neuman en su interpretación. Resultados: el dendograma generó cinco clases intituladas: Rutina Familiar, Atención del Servicio de Salud, Cambios en el Estilo de Vida, Red de Apoyo y Repercusiones Sociales del Cuidado en el Contexto Familiar. Consideraciones finales: las acciones de enfermería con base en la teoría de Betty Neuman promueven subsidios para reconocer elementos que fortalecen las líneas de defensa del sistema familiar. Se pueden explorar estos recursos con el fin de mantener el bienestar y el equilibrio en el contexto del sistema familiar.

Considerações finais: ações de enfermagem fundamentadas na teoria de Betty Neuman fornecem subsídios para o reconhecimento de elementos que fortalecem as linhas de defesa do sistema familiar. Esses recursos podem ser explorados, com vistas à manutenção do bem-estar e o equilíbrio no contexto do sistema familiar.

Objetivo: Descrever a ocorrência da febre pelo vírus Zika (ZIKV) e suas complicações no estado do Tocantins e em sua capital, Palmas. Métodos: Estudo descritivo, utilizando dados dos sistemas de informações em saúde. Resultados: A incidência de casos notificados de febre pelo ZIKV, em 2015 e 2016, foi de 295,2/100 mil e 411,1/100 mil habitantes na população geral, e de 5,9/mil e 27,8/mil nascidos vivos em gestantes, respectivamente. Maiores riscos ocorreram em mulheres, nas idades de 20-39 anos, nos municípios das regiões central e noroeste do estado, durante os meses mais quentes (fevereiro e março). A incidência de microcefalia relacionada à infecção pelo ZIKV na gestação foi de 0,06/mil nascidos vivos. Foi confirmado um caso de síndrome de Guillain-Barré decorrente da infecção pelo ZIKV. Conclusão: A febre pelo ZIKV atingiu o Tocantins intensamente, embora seus desfechos adversos tenham sido menos frequentes que em outros estados.


Objetivo: Describir la aparición de la fiebre del virus del Zika y sus complicaciones, en el estado de Tocantins y en la ciudad de Palmas, su capital. Métodos: Estudio descriptivo utilizando datos de los Sistemas Oficiales de Información. Resultados: La incidencia de casos informados de fiebre por ZIKV en 2015 y 2016 fue 295,2/100.000 y 411,1/100.000 habitantes, respectivamente y 5,9/1000 y 27,8/1000 de nacidos vivos en gestantes, respectivamente. Los riesgos mayores estuvieron en mujeres, entre los 20-39 años de edad, en los municipios de las regiones central y noroeste y en los meses más calurosos (febrero y marzo). La incidencia de microcefalia relacionada con el ZIKV en la gestación fue 0,06/1000 nacidos vivos. Se confirmó un caso de Síndrome de Guillain- Barré debido al ZIKV. Conclusión: La fiebre por el ZIKV afectó intensamente a Tocantins. Pero, sus desenlaces adversos han sido menos frecuentes que en otros estados.


Objective: To describe the occurrence of Zika virus disease and its complications in the state of Tocantins and in its capital, the city of Palmas. Methods: This was a descriptive study using data from health information systems. Results: Incidence of reported Zika virus disease cases in 2015 and 2016 was 295.2/100,000 inhabitants and 411.1/100,000 inhab. in the general population, and 5.9/1,000 and 27.8/1,000 live births, respectively. Higher risks occurred in women, the 20-39 year age group, municipalities in the central and northwestern regions of the state and in hotter months (February and March). Incidence of Zika-related microcephaly during pregnancy was 0.06/1,000 live births. One case of Guillain-Barré Syndrome resulting from Zika virus infection was confirmed. Conclusion: Zika virus disease hit Tocantins intensely, although its adverse outcomes were less frequent than in other states.

La infección por virus Zika es transmitida por la picadura del Aedes sp, aunque también se ha descrito la transmisión transplacentaria a los fetos. La microcefalia congénita es el primer hallazgo relacionado con la enfermedad y se asocia a defectos en la proliferación neuronal y muerte de las células progenitoras corticales que tiene como consecuencia una disminución de la producción neuronal y que resulta en una serie de trastornos cerebrales que comprometen la motricidad, visión, audición y funciones cognitivas. La presente investigación describió las características epidemiológicas, clínicas e imagenológicas en pacientes pediátricos con microcefalia secundario a la sospecha de infección por el virus Zika, que acudieron a la consulta de Neuropediatría del Servicio Desconcentrado Hospital Pediátrico Dr. Agustín Zubillaga de la ciudad de Barquisimeto, estado Lara durante el lapso diciembre 2016- septiembre 2017(AU)


Zika virus infection is transmitted by the bite of Aedes sp, although transplacental transmission to the fetuse has also been described. Congenital microcephaly is the first finding related to the disease and is associated with defects in neuronal proliferation and death of cortical progenitor cells, which result in a decrease in neuronal production and a group of brain disorders which compromise motor skills, vision, hearing and cognitive functions. The present study describes the epidemiological, clinical and imaging characteristics in pediatric patients with microcephaly secondary to suspected Zika virus infection who attended the Neuropediatric consultation of the Servicio Desconcentrado Hospital Pediátrico Dr. Agustín Zubillaga during the December 2016-September 2017 period(AU)

Objetivo: descrever as expansões temporal e geográfica da circulação do vírus Zika (ZIKV) em países e territórios, desde seu isolamento até 2018. Métodos: revisão não sistemática da literatura do período entre 1947 e 2018, utilizando a base MEDLINE e estimativas da Organização Mundial da Saúde. Resultados: desde seu isolamento em 1947, a circulação do ZIKV expandiu-se pela África, Ásia e Pacífico, até chegar à América em 2013, causando manifestações clínicas graves; as maiores soroprevalências foram registradas na ilha de Yap (74%) e no Brasil (63%); mutações genéticas, a ausência de imunidade e a alta susceptibilidade dos vetores podem ter influenciado sua transmissibilidade e ajudam a explicar a magnitude de sua expansão. Conclusão: a expansão da circulação do ZIKV nas Américas foi a mais ampla já registrada, possivelmente resultado de características populacionais e geográficas dos locais por onde o vírus circulou.


Objetivo: Describir las expansiones temporal y geográfica de la circulación del virus Zika en países y territorios, desde su aislamiento hasta 2018. Métodos: Revisión no sistemática de la literatura del período comprendido entre 1947 y 2018 utilizando la base MEDLINE y estimaciones de la Organización Mundial de la Salud. Resultados: Desde su aislamiento en 1947 la circulación del virus Zika se expandió por África, Asia y el Pacífico hasta llegar a América en 2013, causando manifestaciones clínicas graves. Las mayores seroprevalencias se registraron en la isla Yap (74%) y en Brasil (63%). Mutaciones genéticas, ausencia de inmunidad y alta susceptibilidad de los vectores pueden haber influenciado su transmisibilidad y ayudan a explicar la magnitud de su expansión. Conclusión: La expansión de la circulación del virus Zika en las Américas fue la más amplia ya registrada, posiblemente como resultado de características poblacionales y geográficas de los lugares por donde el virus circuló.


Objective: to describe the temporal and geographical expansion of Zika virus (ZIKV) circulation in countries and territories, from the time it was first isolated until 2018. Methods: This was a non-systematic literature review covering the period from 1947 to 2018 using the MEDLINE database and World Health Organization estimates. Results: Since its isolation in 1947, ZIKV circulation spread through Africa, Asia and the Pacific before reaching the Americas in 2013, causing serious clinical manifestations; the highest seroprevalence rates were recorded in Yap (74%) and in Brazil (63%); genetic mutations, absence of immunity and high vector susceptibility may have influenced ZIKV transmissibility and help to explain the magnitude of its expansion. Conclusion: The spread of ZIKV circulation in the Americas was the most extensive recorded thus far, possibly as a result of population and geographical characteristics of the sites where the virus circulated.

Introduction: Microcephaly is a clinical finding that can arise from congenital anomalies or emerge after childbirth. Maternal infections acquired during pregnancy can result in characteristic brain damage in the newborn (NB), which may be visible even in the fetal stage. To describe the epidemiological profile of newborns with reported microcephaly and diagnosed with congenital infections in the state of Rio Grande do Sul between 2015 and 2017. Methods: A cross-sectional study was carried out on data collected from the Public Health Event Registry as well as from medical records. The investigation included serologies for toxoplasmosis and rubella; polymerase chain reaction (PCR) for Zika virus (ZIKV) in the blood and cytomegalovirus in the urine; non-treponemal tests for syphilis; and brain imaging tests. Results: Of the 257 reported cases of microcephaly, 39 were diagnosed with congenital infections. Severe microcephaly was identified in 13 patients (33.3%) and 51.3% of the cases showed alterations in brain imaging tests. In relation to the diagnosis of congenital infections, three patients (7.7%) were diagnosed with ZIKV, nine (23.1%) with cytomegalovirus, nine (23.1%) with toxoplasmosis, and 18 (46.1%) with congenital syphilis. The three cases of ZIKV showed calcification in brain imaging tests, signs of arthrogryposis, excess occipital skin and irritability, characterizing the typical phenotype of ZIKV infection. Conclusions: Most cases of congenital infection had severe neurological lesions, particularly the cases of ZIKV, which can cause neurodevelopmental delays and sequelae in these infants throughout early childhood.

ABSTRACT

Objetivo compreender as vivências de mães que tiveram filhos com microcefalia. Método estudo qualitativo realizado com oito mulheres, maiores de 18 anos. Foi aplicada a técnica da entrevista semiestruturada em abril de 2017. Para a análise dos dados, utilizou-se a técnica de análise de conteúdo. Resultados emergiram três categorias analíticas: a vivência do diagnóstico da microcefalia: ótica de mães; encontros e (des)encontros dos pais e da família no cuidado à criança com microcefalia; aprendizado com os desafios da microcefalia. As falas revelaram a força de vontade e a determinação das mães no enfrentamento das dificuldades, apesar da vivência marcada por expressões de sofrimento e dor. Conclusão a situação vivenciada por mães que tiveram filhos com microcefalia gera incertezas em relação ao futuro do filho, altera a dinâmica familiar, a relação do casal, o rendimento financeiro e determina a opção de abandonar o trabalho profissional para cuidar do filho.


Objetivo comprender las experiencias de las madres que tuvieron hijos con microcefalia. Método estudio cualitativo realizado con ocho mujeres, mayores de 18 años. Se aplicó la técnica de entrevista semiestructurada en abril de 2017. Para analizar los datos, se utilizó la técnica de análisis de contenido. Resultados emergieron tres categorías analíticas: la experiencia del diagnóstico de microcefalia: punto de vista de las madres; encuentros y (des)encuentros de los padres y de la familia en el cuidado del niño con microcefalia; aprendizaje con los desafíos de la microcefalia. Los relatos revelaron la fuerza de voluntad y la determinación de las madres al enfrentar las dificultades, a pesar de la experiencia marcada por expresiones de sufrimiento y dolor. Conclusión La situación vivida por las madres que tuvieron hijos con microcefalia genera incertidumbres relacionadas al futuro del hijo, altera la dinámica familiar, la relación de la pareja, el rendimiento financiero y determina la opción de abandonar el trabajo profesional para cuidar al hijo.


Objective understand the experiences of mothers who had children with microcephaly. Method this is a qualitative study carried out with eight women aged 18 years and older. The semi-structured interview technique was applied in April 2017. For the analysis of the data, the technique of content analysis was used. Results three analytical categories emerged: the experience of the diagnosis of microcephaly: mothers' optics; encounters and disagreements of the parents and the family in the care of the child with microcephaly; learning with the challenges of microcephaly. The speeches revealed the willpower and determination of the mothers in facing the difficulties, despite the experience marked by expressions of suffering and pain. Conclusion the situation experienced by mothers who had children with microcephaly generates uncertainties regarding the future of the child, changes the family dynamics, the relationship of the couple, and the income, and defines the option to abandon the professional work to take care of the child.

Resumen Introducción. El virus del Zika (ZIKV) es un flavivirus con envoltura, transmitido a los seres humanos principalmente por el vector Aedes aegypti. La infección por ZIKV se ha asociado con un gran neurotropismo y con efectos neuropáticos, como el síndrome de Guillain-Barré en el adulto y la microcefalia fetal y posnatal, así como con un síndrome de infección congénita similar al producido por el virus de la rubéola (RV). Objetivo. Comparar las estructuras moleculares de la proteína de envoltura E del virus del Zika (E-ZIKV) y de la E1 del virus de la rubéola (E1-RV), y plantear posibles implicaciones en el neurotropismo y en las alteraciones del sistema nervioso asociadas con el ZIKV. Materiales y métodos. La secuencia de aminoácidos de la proteína E-ZIKV (PDB: 5iZ7) se alineó con la de la glucopreteína E1 del virus de la rubéola (PDB: 4ADG). Los elementos de la estructura secundaria se determinaron usando los programas Vector NTI Advance®, DSSP y POSA, así como herramientas de gestión de datos (AlignX®). Uno de los criterios principales de comparación y alineación fue la asignación de residuos estructuralmente equivalentes, con más de 70 % de identidad. Resultados. La organización estructural de la proteína E-ZIKV (PDB: 5iZ7) fue similar a la de E1-RV (PDB: 4ADG) (70 a 80 % de identidad), y se observó una correspondencia con la estructura definida para las glucoproteínas de fusión de membrana de clase II de los virus con envoltura. E-ZIKV y E1-RV exhibieron elementos estructurales de fusión muy conservados en la región distal del dominio II, asociados con la unión a los receptores celulares de entrada del virus de la rubéola (glucoproteína de mielina del oligodendrocito, Myelin Oligodendrocyte Glycoprotein, MOG), y con los receptores celulares Axl del ZIKV y de otros flavivirus. Conclusión. La comparación de las proteínas E-ZIKV y E1-RV es un paso necesario hacia la definición de otros factores moleculares determinantes del neurotropismo y la patogenia del ZIKV, el cual puede contribuir a generar estrategias de diagnóstico, prevención y tratamiento de las complicaciones neurológicas inducidas por el ZIKV.


Abstract Introduction: Zika virus (ZIKV) is an enveloped flavivirus transmitted to humans mainly by Aedes aegypti. ZIKV infection has been associated with high neurotropism and neuropathic effects such as the Guillain-Barré syndrome in adults, and fetal and postnatal microcephaly and the congenital Zika virus syndrome similar to that produced by rubella virus (VR). Objective: To compare Zika virus membrane protein E (E-ZIKV) and rubella virus membrane protein E1 (E1-RV), and to propose possible implications for neurotropism and nervous system disorders associated with ZIKV infections. Materials and methods: The amino acid sequence of E-ZIKV protein (PDB: 5iZ7) was aligned to that of rubella virus glycoprotein E1 (PDB: 4ADG). The secondary structure elements were determined using the programs Vector NTI Advance®, DSSP, and POSA, and integrated data management tools (AlignX®). One of the main comparison and alignment criteria was the allocation of structurally equivalent residues with more than 70% identity. Results: E-ZIKV structural organization (PDB: 5iZ7) was similar to that of E1-RV (PDB: 4ADG) (70%-80% identity), and it was consistent with relevant structural features of viral membrane class II fusion glycoproteins. E-ZIKV and E1-RV exhibited highly conserved fusion structural elements at the distal region of domain II, which has been associated with the RV myelin oligodendrocyte glycoprotein and Axl cell receptors in ZIKV and other flaviviruses. Conclusion: The comparison of E-ZIKV and E1-RV proteins constitutes an essential step towards the definition of ZIKV neurotropism and pathogenesis molecular determinants, and for the adoption of diagnosis, prevention and treatment strategies against neurological complications induced by ZIKV infection.

Teniendo en cuenta que en los momentos actuales existen emergencias y reemergencias de enfermedades infecciosas, fundamentalmente en zonas tropicales, se impone advertir a los profesionales de la salud y autoridades sanitarias, sobre la amenaza que representa el virus del Zika. A tales efectos se realizó una revisión bibliográfica exhaustiva donde se analizan algunos aspectos de interés, tales como: vectores y reservorios, modos de transmisión, manifestaciones clínicas, diagnóstico diferencial y tratamiento, entre otros, lo cual ayudará a elevar y nivel de conocimientos al respecto y estar preparados ante cualquier epidemia


Taking into account that nowadays emergencies and reemergencies of infectious diseases exist, fundamentally in tropical areas, it is necessary to advice health professionals and health authorities, about the threat that represents the Zika virus. To such effects an exhaustive literature review was carried out where some aspects of interest are analyzed, such as: vectors and pouches, transmission ways, clinical features, differential diagnosis and treatment, among others, which will help to increase the knowledge level in this respect and be prepared to face any epidemic disease of this nature

ABSTRACT

Abstract Introduction: in 2015 an increasing number of congenital microcephaly cases were associated to maternal infection due to Zika virus. Some of these patients presented other alterations and arthrogryposis was the most frequently found. Arthrogryposis is defined as congenital joint contractures involving at least two different areas of the body. Description: arthrogryposis was found in 18 patients with congenital microcephaly due to Zika virus. 67% of the cases were vaginal deliveries. 50% of resuscitation performed in the delivery room was necessary. The mean birth weight was 2.371g, gestational age was 39 weeks and the head circumference was 28.3cm, 15 (83%) of these patients presented severe microcephaly. All the neonates resulted in concomitant hip joints and some also had knees, ankles and wrists affected. Nine neonates (50%) presented an early respiratory distress and four (22%) died due to respiratory failure. Discussion: the neurological result found in patients with Congenital Zika Syndrome seems to be associated to the maternal infection period. During the early stages of embryogenesis, in addition to microcephaly, could be related to the peripheral motor nerves leading to fetal akinesia, joint stiffness and arthrogryposis. These neonates tend to present greater morbimortality with the worst prognosis.


Resumo Introdução: em 2015 observou-se um crescente número de casos de microcefalia congênita associada à infecção materna pelo Zika vírus. Em alguns destes pacientes outras alterações foram encontradas, sendo a mais frequente a artrogripose. Esta é definida como contraturas articulares congênitas envolvendo no mínimo duas diferentes áreas do corpo. Descrição: em 18 pacientes com microcefalia congênita pelo Zika vírus foi encontrada artrogripose associada. A via de parto foi transpelviana em 67% dos casos. A reanimação em sala de parto foi necessária em 50%. A média de peso ao nascimento foi de 2.371g; da idade gestacional de 39,2 semanas e a encontrada para o perímetro cefálico foi 28,3cm, representando microcefalia severa em 15 (83%) pacientes. Todos os neonatos apresentaram acometimento do quadril e em alguns houve comprometimento concomitante das articulações de joelhos, tornozelos e punhos. Nove neonatos (50%) apresentaram desconforto respiratório precoce e quatro (22%) evoluíram para óbito. Discussão: o comprometimento neurológico dos pacientes com Síndrome de Zika congênita parece estar associado ao momento da infecção materna. O acometimento nas fases iniciais da embriogênese, além da microcefalia, pode estar relacionado à lesão de nervos motores periféricos e a um quadro de acinesia fetal, com consequente rigidez articular e artrogripose. Estes neonatos tendem a apresentar maior morbimortalidade, com prognóstico mais desfavorável.

The Zika virus (ZIKV) is an arbovirus which was first isolated in Zika, a valley in Uganda. The first case of human infection was reported in 1954; and since then cases have been described in Africa, Asia, Oceania, and the Americas. Central America has been affected by climate change, poverty, and poor environmental sanitation. Zika virus infection is a vector-borne disease, and there may also be perinatal transmission, as well as by blood transfusions and through infected semen. From a clinical standpoint, ZIKV infection is hardly different Dengue and Chikungunya infections. ZIKV complications are mainly at a neurological level. The governments of the affected regions must have vector control policies, as well as improvements in housing and waste disposal...
